New Open Disclosure Framework for health services

Last updated: 25 June 2026

The Australian Commission on Safety and Quality in Health Care (ACSQH) has published a new Australian Open Disclosure Framework, a tool for communicated unexpected health outcomes and harm.

The framework provides a nationally consistent basis for communicating with consumers, carers and loved ones. It serves as a revision to the previous Open Disclosure Framework published in 2014.

The Framework is endorsed by multiple medical colleges, including the Royal Australian New Zealand College of Psychiatrists (RANZCP).

I encourage all clinicians and healthcare administrators to review this new Framework to ensure that communication with carers and consumers is respectful, transparent and ethically sound.
