Freedom of Information

The Freedom of Information Act 1992 entitles members of the public to request access to records held by a government agency.

The Office of the Chief Psychiatrist manages its own information and records. Records held by the OCP may include:

  • Policies, standards and guidelines
  • Records related to notifiable incidents
  • Information on treatment reported to the Chief Psychiatrist
  • Architectural documents related to authorisations
  • Training and registration information
  • Personnel records
  • General administrative correspondence

The Office of the Chief Psychiatrist does not hold medical records. To request access to medical records, please contact the relevant hospital or service listed on the Department of Health’s website.

Filing a request with the Office of the Chief Psychiatrist for release of information under the Freedom of Information Act 1992

In order to file a request for records under the Freedom of Information Act 1992, please provide the following:

  • An Australian postal address
  • Proof of identity (e.g. a scan of a current Australian passport, driver’s licence or WA Photo Card)
  • A detailed description of the records being requested

Requests can be submitted via email to reception@ocp.wa.gov.au or via mail to:

If the request includes non-personal information, i.e. information about someone other than the person making the request, a fee may be payable.

Once an application has been processed, the Office of the Chief Psychiatrist will provide a response within 45 calendar days.
