Freedom of Information

Under the Freedom of Information Act 1992,  you can access your WA Health medical records. However consideration needs to be given to s.28 of the Freedom of Information Act 1992 in the disclosure of information that may have an adverse effect on the physical or mental health of a patient.

The Office of the Chief Psychiatrist can only  assist if your application is in relation to dealing with this Office, all other applications will need to be directed to the specific health services within Western Australia (please see information below).

Who do I contact?

Each hospital or health service in which you have been a patient holds a separate record of your treatment at that hospital or health service.

For private hospitals or healthcare facilities, you will need to contact the service directly to get a copy of your medical records.

How do I apply for my records from the Office of the Chief Psychiatrist?

Applications to the Office of the Chief Psychiatrist must be in writing addressed to:

Freedom of Information
Office of the Chief Psychiatrist
PO Box 8172
Perth Business Centre WA 6849

You must provide us with the following:

  • An Australian address to which we can mail notices (this is required under the Freedom of Information Act)
  • Sufficient information to clearly identify the documents you are requesting
  • Proof of identity (such as a copy of a valid driver’s licence or birth certificate).
Can I access information about another person?

You cannot access information about another person without their consent.
